Adult Application Changes
Authorization and Disclosure Form Requirement
The revised BSA Adult Applications have included a new authorization and disclosure statement. The new application has an additional page. This will be an "evergreen" statement that allows the BSA to conduct additional criminal background checks in the future. The application is available in English and Spanish.
Previous versions of the applications can still be used if the new authorization and disclosure statement is included with the older form. Click here for the disclosure form.
The definition of a completed BSA adult application will include application, signatures and the applicant’s signature on the authorization and disclosure statement sheet and appropriate fees.
